For Legal DNA Test Samples Following Chain of Custody
An unrelated sample collector with no interest in the test outcome must be present at all times during the sample collection process to observe and attest to the sample collection process.
Collection materials shall not be in the possession of any of the test participants either before or after collection.
ALL test participants, including children, must provide positive identification and submit copies or photos with their samples
Court Admissible Legal DNA Tests require original or legible photocopies of ONE OR BOTH of the following items:
o Government-issued photo identification
o Photograph suitable for positive identification
Immigration, Passport, VISA, and Citizenship DNA Tests require original or legible photocopies of BOTH of the following items:
o Government-issued photo identification
o Photograph suitable for positive identification
The TEST PARTICIPANTS (legal guardian or conservator in the case of a minor or legally incompetent adult) must provide consent on the Client Identification and Consent Form.
The SAMPLE COLLECTOR must complete and sign the Client Identification and Consent Form and sample envelopes. Failure to complete the Collector Statement will invalidate the test.
Once the collection is complete, place all contents in larger envelope and seal securely.
The collector must mail the sample collection kit to the laboratory using a track-able or traceable shipping service.
The samples and paperwork must remain in secure possession of the sample collector at all times prior to shipment.
